Ash was none to happy to be at a big high school party. As far as he was concerned is was just more preppy bull shit. But his best mates, Jackson Red, better known as Jacks, and Viola Branche, better known as Vi, had dragged him along. Apparently there was a rumor going around that some hotshot underground DJ was gonna be spinning at the party, and there were sure to be plenty of drugs drifting around a party this size. Still, this really wasn't Ash's scene, so after snagging a bottle of Jack Daniels from the kitchen, he clambered up the side of the house to the roof.

He liked it up here. He could feel the music rumbling beneath him, not bad, but he sure didn't get the hype if this was the DJ Vi had been so pumped to see. Below was a see of wavering lights, glowsticks spinning around, and above, a sea of black with stars floating across it. Smiling, Ash popped a handfull of pills into his mouth, and chased them down his throat with a big swig from the bottle of Jack. Then he lit up a cigarette, and lay down against the roof, enjoying the view, and the warm buzzing filling his veins.

Ash was halfway through the bottle, three joints, and four more cigarettes deep, when Vi scrabbled up beside him.

"Jesus Ash!" She exclaimed, "Have you been hiding out here by yourself all night?"

Ash shrugged in response, taking another swig from the bottle, before passing it to her. He was definitely intoxicated, but his tolerance was high enough that his head was still clear enough to think, a problem he was seeking to remedy.

"You know I hate those preppy fucks." Ash said, lighting up another smoke.

"Yeah, but it's not just those preppy fucks down there Ash," Vi complained, "It's me and Jacks, and Danny's get a boatload of pill's, and Jace has some seriously quality opiates. Well, he only has a couple pills, he lost at least half somehow."

Ash ducked his head to hide his grin, but not quickly enough. Vi spotted the smile, and her mouth popped open with a surprised laugh.

"You dick!" She squealed, punching him in the arm. "Where you gonna share?"

Ash smiled again and passed her a pill, as he popped another himself. Jacks rolled over the lip of the roof ledge just as they took their pills, and just let himself continue rolling until he came to a stop against Ash and Vi, then lifted his head, demanding with a drunken smile, "Let'sko, pill me mate!"

For a while the three enjoyed the night sky, the music and revelry around them as they got high off stolen goods, and smoked cigarettes stolen from drugstores. Ash was beginning to think this wasn't such a bad party after all, as long as it was just this private little gathering of theirs. It was like they were laughing at the rest of the school, playing out their stupid pre-ordained, normal lives below them, while Ash and Vi and Jacks got drunk on their booze and high on their drugs. It was like they were shoplifting the preppies party, and taking it for themselves. And Ash loved to take from those he didn't find deserving.

Just then, as the three of them puffed cigarettes, and watched the stars, a meteor lit up the sky. Through the buzz of drugs Ash registered shock. This was a low flying meteor, it should've already burned up in the atmosphere, but it was falling, fast, and close.

"SHIT!" Vi swore, sitting up right behind Ash, who has already on his feet, head upturned, watching as plummeted, near enough that it looked like it might take out the entire part with a bulls eye landing. He heard a couple scared yelps from below, but then the meteor was passed, and from his high vantage point, Ash could see it hit the ground, cratering the earth.

But he was already running, jumping off the roof, smirking at the startled yelps of the kids he soared over before hitting the ground, rolling to prevent damage, and continuing in a sprint as he rolled back to his feet. From behind him he could hear Vi and Jacks swearing at him to wait the fuck up and not be an idiot, but he was already outpacing their voices.

The adrenaline buzzing through his body cleared his mind, sobering him up, speeding him onwards, faster and faster, until-

"Shiiiiiit!" Ash swore as he skidded to a halt as he came upon the crater. But his momentum carried him faster, over the edge of the crater, tumbling down, coming to a stop mere inches away from the strange rock. It was hot, but for some reason that his drug and adrenaline addled brain couldn't figure out, it didn't bother him at all. And despite the sprint and fall, he felt fine.

It was probably shock, Ash thought to himself, it's not every day you see a fucking meteor. Standing up and wiping his hands clean on his jeans, he leaned against a boulder sticking out of the side of the crater, and lighting up another smoke.

"Holy fucking meteors Batman." He said, in an awed tone. "I wonder what the hell this thing is?" He voiced his query aloud to the open air. He knew he couldn't have been the only one to see it, hell Jacks and Vi were already on their way after him. He wondered if he should cordon off the area or something, but decided it wasn't his problem, and sank down into his seat again, smoking silently and looking at the massive space junk in awed silence.

New insights into how cells duplicate their DNA

ScienceDaily (Mar. 29, 2012) ? In the beginning ... but how do you begin? That question has long perplexed scientists in fields from cosmology to anthropology. Fortunately, researchers at the Office of Science's Brookhaven National Lab (Brookhaven Lab) are beginning to get at the answer on a small but important scale -- in biology.

Specifically, the researchers looked at how cells begin to duplicate their DNA, so they can then begin to replicate themselves. DNA is the essential stuff of beginnings. Its double strands -- which consist of chemical 'letters' or base pairs -- tell cells how to remake themselves; how to build the protein machines that keep them alive and make them distinct. So before they divide, cells have to duplicate their DNA.

This is a relatively straightforward affair for bacteria (and other simple cells, also called prokaryotes) since they typically only have a single loop of DNA, even though it can be millions of base pairs long. As a consequence, they have just a single point along the strand where the copying starts, called an origin of replication.

However, most of biology that can be seen with the naked eye -- animals and plants and even humble yeast -- is composed of more complicated cells called eukaryotes. Eukaryotes have much more DNA, which is tightly wound into distinct pieces, or spindles, called chromosomes, each of which may have many origins of replication. 'Top gun' cells, say those in humans, have a need for speed, since they have some 3.4 billion DNA base pairs, all of which have to be pulled apart and copied. So in order to finish in a reasonable amount of time, those cells have to begin copying their DNA simultaneously at tens of thousands of different points.

But how do they begin? Namely, how do protein machines find and bind to the right spots along the DNA strands, and then set them up for copying? That's what the team at Brookhaven Lab studied. They used an imaging method known as cryo-electron microscopy to take extremely high resolution images of how the right proteins come together at the right point on the DNA strands, forming a structure called an "origin recognition complex" (ORC). The lab's first-of-a-kind images (taken using yeast cells, which are also eukaryotes), showed how the shape of the complex changes as it sets the DNA up for duplication.

Scientists then gathered additional details about individual parts of the structure from previously made X-ray crystallography images, which showed the positions of many individual portions of the complex. Then they took all of their information and ran a detailed computer simulation, which gave scientists a good idea of how the whole process works.

That's important since beginnings can go bad, and uncontrolled cell division is the hallmark of many cancers. The new insights from Brookhaven Lab might lead to new ways to attack cancers at a basic level, one reason that the research was also supported by the National Institutes of Health.

But there's also a deeper reason to do basic research. Beginnings are one of the most precious opportunities of all, the chance to create, to discover. That's what happens with each new experiment run at National Laboratories supported by the Office of Science: Each day it's a new chance to begin again?in the beginning.

What is the difference between sociology, anthropology and ...

What is the difference between sociology, anthropology and archeology? What type of careers do these degrees lead to? Do people usually have to get a phd or masters if they pursue these fields?

In addition to the other good answers ? in the English-speaking world, there is a different emphasis in Anthropology, as defined in the USA and in the Commonwealth countries.

The American interpretation of Anthropology tends to be more eclectic; and includes prehistory, archaeology, and biological and ecological factors in explaining human society.

In the British and Commonwealth traditions, these factors are not discounted by any means but are given somewhat less emphasis. The primary focus of Anthropology is human society and social relations ? with commensurate attention to economics, politics and history as determining factors and objects of study.

Neither approach is right or wrong. They developed that way for complex and interesting reasons in the history of the discipline. But the upshot ? in the US, there is more cross-over between anthropology, prehistory and archaeology. In some Commonwealth departments, Anthropology is almost indistinguishable from Sociology.

Sociology, Anthropology and Archaeology are all fascinating fields, which can give you insights into teh very essence of human existence. Whichever one you choose, a rewarding career is possible.

Sociology ? The study of society, or groups of humans.

Anthropology ? The study of humans and our primate ancestors. In all aspects (physical, cultural, linguistics, archaeology). Everything from human evolution, fossil record, skeletal biology, cultures, how language developed, non-human primate behavior, biology, and evolution, to ancient artifacts.

Archaeology ? The study of human artifacts (it?s a sub-field of Anthropology). Artifacts are objects that humans have created or used in some way (like using animal bones as tools, etc). It?s not about finding fossils or bones, those are paleontologists.

TiVo sues Time Warner Cable, Motorola Mobility in fresh patent assault

Four days ago TiVo abandoned its protracted patent litigation against Microsoft and we sighed in relief at the idea that people were learning to just get along. Our respite was short lived, however, when the company revealed it was lobbing litigation-shaped grenades over the fence toward Motorola Mobility and Time Warner Cable. The lawsuit, filed in the Patent Troll haven of Texas' Eastern District, concerns patents 6,223,389, 7,529,465 and 6,792,195. The first of that trio is the contentious Time Warping patent that's been argued over so much we'd like to slice it in two, King Solomon style. Details beyond that are few and far between, but something tells us we'll be hearing more about the tiff in the future... whether we want to or not.

In an age of war and reality television, ?The Hunger Games? depicts how two underdogs struggle to survive a deadly competition between teenagers. The buzz around the hot sci-fi film has drawn massive attention among social media, critics, and fans, all questioning if it will live up to the hype and reach the same levels of success as the Harry Potter and Twilight franchises.

At the Capitol, a hedonistic place filled with flamboyantly dressed citizens, the pair meets unique characters, like the charismatic television host Caesar Flickerman (Stanley Tucci) and the benevolent stylist Cinna (rock musician Lenny Kravitz).

An expert archer, Katniss relies on her sharp instincts and mentorship of the impaired former District 12 victor, Haymitch (Woody Harrelson), to battle and outsmart the twisted game. Katniss struggles with loyalties in the game arena and spirals further into conflict when her fellow tribute, Peeta, is the last person she wants to kill.

Lawrence is a natural artist embodying the fierce and determined Katniss, who has become hardened and untrusting over time to everyone except Prim and her best friend Gale (Liam Hemsworth). In a film that is vastly centered on her character?s perspective, Lawrence?s performance and energy alone outshone many other aspects of the film, providing heart-pounding entertainment and perfectly capturing the essence of Katniss.

Lawrence has garnered an Oscar nomination for Best Actress for her performance in the low-budget indie thriller Winter?s Bone and is becoming one of Hollywood?s most promising actresses. Lawrence?s co-star, Hutcherson, portrays the extremely good-hearted Peeta who is secretly in love with Katniss, though that is not the focused element of the film. He and Lawrence work off each other fluently onscreen, but his presence is faint.

The Hunger Games features a brilliantly versatile cast including Elizabeth Banks (Seabiscuit), Lenny Kravitz (Precious), Stanley Tucci (The Devil Wears Prada, Julie & Julia), and Toby Jones (Tinker Tailor Soldier Spy).

Donald Sutherland (Space Cowboys) makes his appearance as the Capitol?s dictator, President Snow, and Wes Bentley (American Beauty) as the Head Gamemaker, both controlling all aspects of the game.

The film stays close to the novel with consistent character development, cinematography, and a terrific cast, allowing this film to be a satisfying book-to-film translation. Much of this credit goes to director Gary Ross who co-wrote the screenplay with the novel?s author, Suzanne Collins.

Ross?s clever use of facial close-up shots tugs at the heart without relying on clich?s or extra gimmicks, fully portraying Katniss?s first-person standpoint without the help of narration while his shaky-cam shots give the rawness that fits the content.

The only issue holding back half a star was the special effects, more specifically, the treatment of fire in the movie. Given that fire is a common element throughout the story, the outcome of the flames is slightly disappointing. Additionally, there were some minor elements from the book that were altered to make way for the bigger sequences and motifs of the story but not distracting enough to dispute that the film fully complements the book.

Despite the constraints on fully depicting the violence and gory scenarios from the book due to the film?s PG-13 rating, Ross flawlessly delivers Katniss?s point of view, the terrifying battles fought in the arena, and a number of deaths without neglecting emotions. The film?s soundtrack, or rather, lack thereof, magnifies each moment, creating a greater impact to the overall tone and evoking a more compelling response from the audience.

The Hunger Games successfully portrays Suzanne Collins?s heavyweight message of sacrifice and survival, sparking the strong beginning of a new cultural phenomenon and definitely living up to the hype.

As the world?s best poets and songwriters have expressed through the ages, love is a powerful thing. But did you know that adult love?which is our emulation of the mother-child bond?could actually lower your blood pressure, improve your health and lessen physical pain?

A startling piece in the New York Times yesterday explored the many ways that the brain and the human body respond to love, revealing that after a baby is born and forms an incredibly close bond with its mother, that baby grows up and spends the rest of its years searching for the adult equivalent of the mother-child bond. And once that adult finds a loving, supportive partner, the relationship is even more beneficial than most of us probably realize.

Diane Ackerman, the writer of the Times story, describes a 2006 experiment done by University of Virginia neuroscientist James Coan in which he gave an electric shock to the ankles of women in happy, committed relationships and measured their anxiety and pain. Then he gave them the same amount of shock while they were holding their husband?s hand and found that their anxiety and pain was significantly lower. Women who were in troubled relationships weren?t soothed by holding their husband?s hand.

?A happy marriage relieves stress and makes one feel as safe as an adored baby,? Ackerman writes in the Times.

But just as love can be overwhelmingly joyful and soothing, as many of us know all too well the end of a love relationship can be so devastating that we feel actual physical pain.

?Whether they speak Armenian or Mandarin, people around the word use the same images of physical pain to describe a broken heart, which they perceive as crushing and crippling,? Ackerman writes. ?It?s not just a metaphor for an emotional punch. Social pain can trigger the same sort of distress as a stomachache or a broken bone.?

If we spend our lives searching for the adult equivalent of the bond we had with our mothers, it makes me wonder what happens to those of us who didn?t have the benefit of a strong motherly bond when we were infants. I don?t think a strong mother-child bond is something we can take for granted?not every new mother is in the position or emotional state to create that special relationship. So if it?s not there, how does it affect that child?s love relationships down the road?

These are questions that don?t have easy answers, but it does point to an immutable fact: the mother-child bond is perhaps the most important human relationship we will ever experience. Everything else is just our desperate attempt to simulate it, as often as we can.

Value Investing and Value Stocks Explained | Safe Investment Plan

The value investor adheres to the principle of buying only undervalued stocks ? undervalued in the sense that the stock?s current price fails to reflect (as far as the investor is concerned) its ?fair? market price or its true ?intrinsic worth?. Famous advocates of the value investing philosophy include the legendary and very much alive investor?Warren Buffet, and the lateBenjamin Graham?- one of the first proponents of value investing, a subject he taught as a professor at the?Columbia Business School?in 1928.

The overriding reason why value investors seek out undervalued stocks is because value stocks tend to offer a higher degree of capital preservation than growth stocks. Value investors are not so much concerned with how much they might make out of an investment, but how much of their capital they could lose ? i.e. having bought a stock, what are the chances of the price falling never mind rising?

What?s a stock worth?

Depending on?when?and?where?you look ? and even if the business appears to be totally sound and is making money ? it?s not particularly difficult to find stocks where, for one reason or another, the stock price fails to reflect the intrinsic worth of the business. But how can a value investor establish a company?s true intrinsic worth? In other words, how does the value investor pinpoint an undervalued company?

It?s all in the numbers

Essentially, value investors use cold, hard, quantifiable historical data to determine whether a stock is undervalued or not. The experienced value investor will analyze a range of the businesses? financial fundamentals such as the price-earnings ratio (P/E), earnings yield, discounted cash flow analysis (DCF) and price-to-book ratios ? to name but four of the nine+ key fundamental ratios. The numbers that emerge from that quantitative analysis provide a reasonably accurate indication of the company?s real worth and whether its shares are fairly valued or not. If a stock?s fair value is higher than its current market price, then that stock might be a value stock ? assuming of course that there are no obvious reasons why the price is lower than it ought to be.

Why stocks are undervalued

Assuming the stock doesn?t warrant the cold shoulder from investors, stocks can be undervalued because they?re not particularly popular with the investors at that moment of time, or simply because the stock is off the market?s radar. Even if the fundamentals add up, a stock can deserve to be undervalued because of disappointing results, a poor credit rating, management changes, a scandal of some kind, the business is unfashionable, or there are problems relating to the company?s products or services. Where those circumstances exist, and the stock price is lower than the fundamentals suggest it ought to be, that stock is sometimes deemed to be a ?Value Trap?.

Comparing apples with apples

It is also possible for two investors to analyze the same fundamentals and each come to a different conclusion regarding the intrinsic value. If however each investor calculated the values applying?Benjamin Graham?s?principles ? where the focus is totally on documented historical numbers ? both individuals would reach the same number.

About the Margin of Safety

By purchasing a stock which is priced at less than its real worth, the chances of the price falling much further are relatively low and as such the investor?s capital is less exposed to risk. For that reason, value stocks are considered to offer a ?Margin of Safety? ? the higher the MoS, the better protected the investors capital is judged to be. As mentioned previously, it can be extremely difficult to calculate accurately a stock?s intrinsic worth, so a reasonable Margin of Safety (MoS) can shield the investor from the adverse effects of incorrect calculations, a market downturn, or both. For large cap, blue chip and highly liquid stocks, and having established the stock?s intrinsic value, the value investor would hope to purchase that stock at a 90% discount to its intrinsic value ? i.e. a 10% MoS: more speculative, smaller or illiquid stocks should ideally be bought at a discount of 50%+ to their intrinsic value, thus providing a 50% MoS.

The attractions of value investing

? The MoS can provide an element of capital preservation
? Value investing is a single minded and highly disciplined approach: Value investors make their investment decisions based on cold, hard facts, rather than hype, fashion, trends or human emotions
? The returns: In 1984, having examined the performance of investors who worked at?Graham-Newman Corporation?and were thus most influenced by?Benjamin Graham,?Warren Buffett?concluded that as a doctrine, value investing is, on average, successful in the long run

The disadvantages of value investing

? Value investors must be prepared to miss out on short term investment opportunities
? Value investing requires willpower. Value investors buy when other people are selling and sell when other people are buying, which can pose psychologically difficulties for some investors
? Value investing demands patience ? essentially it?s a ?buy and hold? strategy
? The ?value trap?: a stock may be undervalued not just because it?s out of favour with the market but because it deserves to be
? The importance or relevance of more qualitative analytical factors such as the abilities of a company?s management or the value of its brands or goodwill are not taken into account

Relaxation: Simple Breathing Exercise

I've been out of hospital a week now and I remember the doctors telling me before I left that relaxation was good and stress was not. I've been taking it easier than normal this week but I realise that I need to compensate using other tactics. I gave this breathing exercise a try today (thanks to +Mitch Joel for posting about it) and I found that it really helped, so I'm going to carry on doing it morning and night and whenever I feel stress brewing.

This is the 4 ? 7 ? 8 breathing exercise:

Step 1: breathe out. Let it all out.

Step 2: breath in through your nose and fill your belly with air (let your belly push out) for 4 seconds.

Step 3: hold you breath for 7 seconds.

Step 4: release your breath slowly through your mouth for 8 seconds. Make sure that your tongue is resting behind your top middle teeth and you should pucker you lips and make a "blowing out" sound.

Step 5: repeat this 4-6 times.

Mysterious cloud spotted on Mars

Amateur astronomers are puzzling over a seemingly anomalous cloud that has shown up on images of Mars taken over the past few days. Is it really a cloud, or a trick of the eye? Does it really extend 150 miles up from the surface, as some of the observers suggest? And what churned up all that stuff, anyway? The amateurs and the pros will be trying to resolve those questions before the phenomenon fades away.

"It's not completely unexpected," Jonathon Hill, a member of the team at the Mars Space Flight Facility at Arizona State University, told me today. "But it's bigger than we would expect, and it's definitely something that our atmosphere guys want to take a look at."


Hill and his colleagues will be looking at the area where the cloud was spotted using the Thermal Emission Imaging System, or THEMIS, which is one of the instruments on NASA's Mars Odyssey orbiter.

"In the command upload we're preparing to send today, we've included observations that will hopefully capture some of these recent clouds," Hill wrote in an email. "Our THEMIS camera on Mars Odyssey is capable of acquiring simultaneous visible and thermal infrared images, so our atmospheric researchers are pretty excited about the possibility of not only getting a good look at the cloud structures, but also their temperatures."

THEMIS will be checking out heightened cloud activity around Mars' shield volcanoes as well as around the southern site spotted by the amateurs. Pictures from a camera on NASA's Mars Reconnaissance Orbiter, called the Mars Color Imager, or MARCI, might provide further clues about the southern cloud feature. And amateur astronomers are sending out the alert for observers to keep a close watch on the Red Planet over the coming days.

There's been lots of buzz about the high-altitude cloud on Cloudy Nights and other online discussion forums for skywatchers. Sky & Telescope's Sean Walker says the puff of white was first noticed on March 20 by Wayne Jaeschke, an amateur astrophotographer from Pennsylvania. Since then, other observers have identified the feature in images going back as far as March 12.

All sorts of hypotheses have been proposed: Could it be debris kicked up by a meteoric impact? Is it a huge weather system? Is it merely a funny kind of glint caused by a combination of lighting and atmospheric conditions?

In an email, Jaeschke told me that the feature is "still there, although it has decreased in size over the past two days."

"This has led some to believe that it was some sort of transient-type event," Jaeschke said. So it's crucial to make as many observations of the area as possible over the next few days.

Walker says the feature is currently well-placed for viewing from the Americas. He says it should show up on the edge of Mars' disk around 1:10 a.m. ET Saturday, and 39.5 minutes later on each succeeding night. Consult the photos above for guidance on where to point a medium-size telescope ? keeping in mind that these images are inverted to appear as they would through a telescope, with south pointing "up." Arizona State University's Hill says the area in question is called Terra Cimmeria.

Observation reports should be sent via email to Richard McKim, director of the British Astronomical Association's Mars Section. (The linked website includes McKim's email address, as well as a picture of the chap.)

Solving this mystery ? if it indeed turns out to be an honest-to-goodness mystery rather than a mere quirk?? may require additional data from the big guns of the astronomy world. But in any case, the episode illustrates once again how much amateurs can contribute to uncovering the wonders of the cosmos.

"When it comes to Mars, amateurs and professionals working together give you way more insight into ongoing processes, because with so many amateurs, you're continuously monitoring changes in the planet," Hill told me. "They provide a perspective and a context that we don't usually get."

Google Voice updated, brings folders and dialer integration for ICS

If you're a Google Voice user there's quite the update waiting for you in the Google Play store this afternoon. The newest version (0.4.2.54) features tight integration with the Ice Cream Sandwich dialer, a new voicemail option, and the inclusion of labels (think folders) for your inbox. Here's the not-so-fun part -- unless you have a phone with Ice Cream Sandwich, you won't get to see most of it.

All phones will get the inbox labels, which is one thing many folks, myself included, have been waiting for. If you use your Google Voice inbox as a catch-all storage and never delete anything you can now attempt to keep things organized and access it all from your phone. This is the best change for me, but the others are nice as well.

Voicemails are now able to be played through the native Ice Cream Sandwich dialer, just like carrier voicemail. You can adjust the speed to play them faster or slower (handy when skipping to the good part). You enable this in the Google Voice settings, and it's a great way to manage two numbers if that's how you roll. That is how I roll, and it's really handy. Too bad it's ICS only. HYSTERECTOMY SURGICAL REMOVAL OF UTERUS ? Op. Dr ...


Hysterectomy is the surgical removal of all or part of the uterus. In a total hysterectomy, the uterus and cervix are removed. In some cases, the fallopian tubes and ovaries are removed along with the uterus, which is a hysterectomy with bilateral salpingo-oophorectomy. In a subtotal hysterectomy, only the uterus is removed. In a radical hysterectomy, the uterus, cervix, ovaries, oviducts, lymph nodes, and lymph channels are removed. The type of hysterectomy performed depends on the reason for the procedure. In all cases, menstruation permanently stops and a woman loses the ability to bear children. Purpose
The most frequent reason for hysterectomy in American women is to remove fibroid tumors, accounting for 30% of these surgeries. Fibroid tumors are non-cancerous (benign) growths in the uterus that can cause pelvic, low back pain, and heavy or lengthy menstrual periods. They occur in 30?40% of women over age 40, and are three times more likely to be present in African-American women than in Caucasian women. Fibroids do not need to be removed unless they are causing symptoms that interfere with a woman?s normal activities.

Treatment of endometriosis is the reason for 20% of hysterectomies. The endometrium is the lining of the uterus. Endometriosis occurs when the cells from the endometrium begin growing outside the uterus. The outlying endometrial cells respond to the hormones that control the menstrual cycle, bleeding each month the way the lining of the uterus does. This causes irritation of the surrounding tissue, leading to pain and scarring.

Twenty percent of hysterectomies are done because of heavy or abnormal vaginal bleeding that cannot be linked to any specific cause and cannot be controlled by other means. Another 20% are performed to treat prolapsed uterus, pelvic inflammatory disease, or endometrial hyperplasia, a potentially pre-cancerous condition. About 10% of hysterectomies are performed to treat cancer of the cervix, ovaries, or uterus. Women with cancer in one or more of these organs almost always have the organ(s) removed as part of their cancer treatment.

Demographics
Hysterectomy is the second most common operation performed on women in the United States. About 556,000 of these surgeries are done annually. By age 60, approximately one out of every three American women will have had a hysterectomy. It is estimated that 30% of hysterectomies are unnecessary.

The frequency with which hysterectomies are performed in the United States has been questioned in recent years. It has been suggested that a large number of hysterectomies are performed unnecessarily. The United States has the highest rate of hysterectomies of any country in the world. Also, the frequency of this surgery varies across different regions of the United States. Rates are highest in the South and Midwest, and are higher for African-American women. In recent years, although the number of hysterectomies performed has declined, the number of hysterectomies performed on younger women aged 30s and 40s is increasing, and 55% of all hysterectomies are performed on women ages 35?49.Description A hysterectomy is classified according to what structures are removed during the procedure and what method is used to remove them.

Total hysterectomy
A total hysterectomy, sometimes called a simple hysterectomy, removes the entire uterus and the cervix. The ovaries are not removed and continue to secrete hormones. Total hysterectomies are usually performed in the case of uterine and cervical cancer. This is the most common kind of hysterectomy.

In addition to a total hysterectomy, a procedure called a bilateral salpingo-oophorectomy is sometimes performed. This surgery removes the ovaries and the fallopian tubes. Removal of the ovaries eliminates the main source of the hormone estrogen, so menopause occurs immediately. Removal of the ovaries and fallopian tubes is performed in about one-third of hysterectomy operations, often to reduce the risk of ovarian cancer.

Subtotal hysterectomy
If the reason for the hysterectomy is to remove uterine fibroids, treat abnormal bleeding, or relieve pelvic pain, it may be possible to remove only the uterus and leave the cervix. This procedure is called a subtotal hysterectomy (or partial hysterectomy), and removes the least amount of tissue. The opening to the cervix is left in place. Some women believe that leaving the cervix intact aids in their achieving sexual satisfaction. This procedure, which used to be rare, is now performed more frequently. Subtotal hysterectomy is easier to perform than a total hysterectomy, but leaves a woman at risk for cervical cancer. She will still need to get yearly Pap smears.

Radical hysterectomy
Radical hysterectomies are performed on women with cervical cancer or endometrial cancer that has spread to the cervix. A radical hysterectomy removes the uterus, cervix, above part of the vagina, ovaries, fallopian tubes, lymph nodes, lymph channels, and tissue in the pelvic cavity that surrounds the cervix. This type of hysterectomy removes the most tissue and requires the longest hospital stay and a longer recovery period.

Methods of hysterectomy
There are two ways that hysterectomies can be performed. The choice of method depends on the type of hysterectomy, the doctor?s experience, and the reason for the hysterectomy.

ABDOMINAL HYSTERECTOMY. About 75% of hysterectomies performed in the United States are abdominal hysterectomies. The surgeon makes a 4?6-in (10?15-cm) incision either horizontally across the pubic hair line from hip bone to hip bone or vertically from navel to pubic bone. Horizontal incisions leave a less noticeable scar, but vertical incisions give the surgeon a better view of the abdominal cavity. The blood vessels, fallopian tubes, and ligaments are cut away from the uterus, which is lifted out. Abdominal hysterectomies take from one to three hours. The hospital stay is three to five days, and it takes four to eight weeks to return to normal activities.

The advantages of an abdominal hysterectomy are that the uterus can be removed even if a woman has internal scarring (adhesions) from previous surgery or her fibroids are large. The surgeon has a good view of the abdominal cavity and more room to work. Also, surgeons tend to have the most experience with this type of hysterectomy. The abdominal incision is more painful than with vaginal hysterectomy, and the recovery period is longer.

VAGINAL HYSTERECTOMY. With a vaginal hysterectomy, the surgeon makes an incision near the top of the vagina. The surgeon then reaches through this incision to cut and tie off the ligaments, blood vessels, and fallopian tubes. Once the uterus is cut free, it is removed through the vagina. The operation takes one to two hours. The hospital stay is usually one to three days, and the return to normal activities takes about four weeks.The advantages of this procedure are that it leaves no visible scar and is less painful.

The disadvantage is that it is more difficult for the surgeon to see the uterus and surrounding tissue. This makes complications more common. Large fibroids cannot be removed using this technique. It is very difficult to remove the ovaries during a vaginal hysterectomy, so this approach may not be possible if the ovaries are involved. Vaginal hysterectomy can also be performed using a laparoscopic technique. With this surgery, a tube containing a tiny camera is inserted through an incision inthe navel. This allows the surgeon to see the uterus on a video monitor. The surgeon then inserts two slender instruments through small incisions in the abdomen and uses them to cut and tie off the blood vessels, fallopian tubes, and ligaments.

When the uterus is detached, it is removed though a small incision at the top of the vagina. This technique, called laparoscopic-assisted vaginal hysterectomy, allows surgeons to perform a vaginal hysterectomy that might otherwise be too difficult. The hospital stay is usually only one day. Recovery time is about two weeks. The disadvantage is that this operation is relatively new and requires great skill by the surgeon. Any vaginal hysterectomy may have to be converted to an abdominal hysterectomy during surgery if complications develop.
